Changes can not only be felt but also measured: which blood markers respond first to a healthier lifestyle?

The impact of lifestyle changes should be assessed comprehensively – evaluating overall well-being, body weight changes, blood pressure, while also monitoring glucose, lipid levels, or inflammation markers.

According to Evaldas Šivickas, a family doctor at the health solutions center „Antėja“, glucose concentration fasting and triglycerides are most likely to respond to healthier habits. These indicators are influenced not only by long-term lifestyle but also by recent days’ diet, physical activity, alcohol consumption, sleep quality, and stress experienced.

“If a person is overweight, insulin resistant, has elevated triglycerides or metabolic fatty liver disease, consistent lifestyle changes can be noticeable after just a few weeks,” says E. Šivickas.

What can change in 30 days?

Within one month, fasting glucose concentration, triglyceride levels, and in some cases – ALT or GGT indicators may decrease. A slight reduction in low-density lipoproteins, or LDL cholesterol, may also be observed.

The magnitude of results depends on the initial health status, chosen diet, physical activity, body weight changes, and genetic factors. However, one month is usually too short to assess the final impact of lifestyle changes. During this time, markers like ferritin or homocysteine usually change more slowly, so their changes are more meaningful to evaluate over a longer period.

“After three months, it is possible to more reliably assess glycated hemoglobin (HbA1c), lipid, and liver enzyme values. During this period, not only a short-term body response is visible but also whether new habits were maintained,” explains the doctor.

HbA1c reflects the average glucose concentration over about two to three months, making this marker more suitable for monitoring long-term changes.

Glucose and triglycerides change first

Triglycerides and fasting glucose usually respond first to lifestyle changes.

“These markers are quite sensitive to reduced sugar, rapidly absorbed carbohydrates, and alcohol intake or increased physical activity,” says E. Šivickas.

Liver enzyme changes can also be quite rapid if their elevation was related to alcohol consumption, fatty liver, or metabolic disorders.

LDL cholesterol usually changes over several weeks, but its change depends more on long-term dietary habits. The intake of saturated fats and fiber, as well as body weight, are important.

High-density lipoproteins, or HDL cholesterol, values usually change more slowly and less markedly.

Why can glucose levels change quickly?

Fasting glucose test shows glucose concentration at the specific time of the test. Therefore, this marker can change even after a few weeks of healthier eating or increased physical activity.

However, short-term circumstances can also affect results: poor sleep, stress, acute illness, physical exertion the previous day, or insufficient fasting time before the test.

Triglycerides are not a type of cholesterol

When discussing lipid markers, it is important to distinguish cholesterol fractions from triglycerides. Although triglycerides are assessed in a lipid panel and often respond fastest to lifestyle changes, they are not a type of cholesterol.

Triglyceride concentration can fluctuate significantly depending on recent days’ diet, alcohol consumption, physical activity, and even test conditions.

When assessing long-term cardiovascular risk, one of the most important markers remains LDL cholesterol. It better reflects the amount of atherogenic lipoproteins that can accumulate in blood vessel walls than total cholesterol.

Non-HDL cholesterol is also useful. It is calculated by subtracting HDL cholesterol from total cholesterol, includes more atherogenic particles, and is used to assess cardiovascular disease risk.

When assessing overall cardiovascular risk, additional information can be provided by hsCRP and homocysteine tests. The first helps evaluate low-grade inflammation in the body, and the second is one of the independent atherosclerosis risk markers, which can be influenced not only by genetic factors but also by other conditions, e.g., deficiency of Vitamin B6, B12, or folic acid.

Better well-being can appear earlier

A person may feel better even before lifestyle changes are clearly reflected in blood tests.

“Improved sleep, more regular eating, increased physical activity, or reduced stress can quite quickly improve energy levels, work capacity, mood, digestion, or overall well-being,” lists E. Šivickas.

Therefore, when evaluating the benefits of healthier habits, one should not limit to laboratory numbers alone. Changes in body weight, waist circumference, blood pressure, physical fitness, and daily well-being are also important.

A complete blood count or ferritin level can provide important additional information about health status, and electrolytes – sodium, potassium, chloride, magnesium, and calcium – allow assessment of fluid balance and processes important for normal nerve and muscle function.

Test results can be distorted by even one workout

According to the doctor, before evaluating blood test results, it is important to remember that they can be affected by short-term circumstances.

One intense workout can temporarily increase creatine kinase (CK), AST, and ALT concentrations. Such changes can usually last for several days. Therefore, elevated AST or ALT after intense exercise does not always indicate liver damage.

Alcohol can affect triglyceride, glucose, and liver enzyme results. Poor sleep can temporarily reduce insulin sensitivity and affect glucose metabolism, and food eaten the day before usually has the greatest impact on triglycerides.

Therefore, to objectively compare results of different tests, they should be performed under as similar conditions as possible.

When is it worth repeating tests?

If there are no very significant deviations or other medical reasons to perform tests earlier, it is usually reasonable to repeat them after three or four months. This period is generally sufficient to assess more stable changes in glucose, lipids, and other program-evaluated markers.

If the initial marker was very elevated, new symptoms appeared, treatment with medications started, or disease is suspected, the timing of test repetition should be chosen individually.

According to the doctor, the biggest mistake is to evaluate a single laboratory number as a final result and ignore circumstances. A one-time change can be caused by sleep, stress, physical exertion, infection, alcohol, medications, or other factors.

“The impact of lifestyle is best assessed by trend, comparing several tests over time, performed under similar conditions, while also evaluating body weight, waist circumference, blood pressure, physical fitness, and well-being,” emphasizes E. Šivickas.

The goal of a healthier life should not be limited to better blood test results. The greatest health benefit comes from consistent habits that a person can maintain for a long time.
