Vilnius and San Francisco-based “Spike” has created AI agents capable of voice communication and independently performing complex tasks that help hospitals improve healthcare services for patients. After the company’s acquisition, Spike’s solution will be immediately used for autonomous Revenue Cycle Management (RCM) in rehabilitation and physical therapy facilities in the US.

The Spike team in Lithuania developed what other market players were only considering – voice-communicating AI agents that think and act autonomously. We are investing in this team and in Lithuania as a long-term hub for our AI engineering,” said Nick Hedges, head of Raintree.
The AI agents created by Spike understand the RCM context, make decisions, and independently perform multi-step tasks – calling insurance companies, handling customer inquiries, performing insurance authorization functions, and communicating with patients.
This technology should help users of the Raintree platform – hospitals and clinics providing rehabilitation services in the US – efficiently manage the entire revenue cycle, from managing patient visit schedules and health insurance verification to payment collection. It is expected that the solution will drastically reduce the administrative burden on hospital staff, nurses, and doctors, and accelerate payments for healthcare services. This allows doctors to spend more time on direct patient care.

P. Gudžius, together with partner Nikita Pajanok, founded Spike in 2022. The company formed its engineering core in Vilnius and developed its commercial activities from San Francisco – this structure allowed it to compete for global healthcare sector clients by leveraging Lithuanian technology talent.

“We wanted to prove that a team based in Vilnius can create an AI solution that will become a market standard. Our company’s acquisition confirms that this strategy has worked,” said Nikita Pajanok, co-founder and technology head of Spike.
Spike’s investors include Silicon Valley and EU venture capital funds, among them Practica Capital, The Venture City, Plug & Play, Geek Ventures, CEAS Investments, APX, and the NVIDIA Inception program. The company operates from Vilnius and San Francisco and is recognized as one of the fastest-growing health AI startups in the Baltic countries.
The company’s engineering team consists of professionals in their field – including PhDs in machine learning (ML), Cambridge University graduates, as well as specialists who have worked at Google, Bloomberg, Oracle, and Amazon One Medical.
Raintree plans to continue expanding its operations in Lithuania. The goal is for Vilnius to become the main hub for AI engineering talent attraction in the Baltic countries.
